1) Construct your own Questionnaire: You have convinced your supervisor that Ms. Mary’s questionnaire is so bad, that a new one is needed. So of course, she has given you the task of drafting a new questionnaire. The purpose of the questionnaire is the same – to get a general picture of the number and type of incidents at the school. You may choose a time period that makes sense to you. However, it is important to try to identify duplicate data, so construct your questionnaire carefully. Your questionnaire should include at a minimum: A cover letter explaining the purpose of the questionnaire and assuring the participants that answers will be kept in strictest confidence A survey that includes the following: A minimum of 4 closed questions A minimum of 2 open questions In a brief separate essay, discuss: How would you pilot your questionnaire to find out any problems, so they can be corrected before sending it out to everyone? Explain your response. How would you distribute the questionnaire? Explain your response.
